What the result can and cannot say
The shown timed, peripheral steady-state samples reproduce AUC24 ≈416 mg·h/L under the stated one-compartment first-order intermittent-infusion model; they do not prescribe a dose change.
A trough outside an older range is not independently decisive when AUC-guided vancomycin monitoring is used. A serial tacrolimus trend changes meaning if the method changes, and a total phenytoin result can mislead when binding is abnormal. Method-specific reference intervals, reportable ranges, critical limits, and escalation rules require local verification.
State exactly what the timed, method-specific result supports and what still depends on local policy or clinical judgment.
