Choosing the test that matches the question

An assay output supports only the stated analytical measurement. It does not independently prove adherence to a prescribed regimen, confirm absorption, rule in or out a drug interaction, or answer a procedural bleeding-risk question. Before selecting a test, separate what is actually being asked: is this a drug-exposure question (was the drug given or absorbed), a pharmacodynamic-response question (is the intended platelet pathway blocked), an adherence question (did the patient take it as prescribed), or a procedural-risk question (should a planned procedure proceed)? Only the pharmacodynamic-response question has a laboratory test family that addresses it directly, and even that test family answers a narrower slice of the question than any of the other three.

For this case, the stated question, residual ADP-pathway reactivity, calls for an ADP-agonist method: ADP-stimulated LTA or an ADP/P2Y12-specific cartridge assay. Arachidonic-acid-induced LTA would answer a COX-1/thromboxane question, not this one. Routine PT, aPTT, and platelet count do not address either pathway. Timing relative to dose, drug initiation, discontinuation, and specimen collection can all change a pharmacodynamic result and must be recorded before interpretation begins, because a result drawn at the wrong point relative to dosing can look falsely reassuring or falsely abnormal.

Current evidence does not support a universal role for routine platelet-function or genetic testing across all patients. The 2024 percutaneous coronary intervention (PCI) consensus supports selective, context-specific testing considered only for defined clinical questions, not a blanket testing strategy applied to every antiplatelet drug, indication, or procedure. That boundary matters as much as the mechanism: knowing which pathway a drug affects does not by itself justify ordering a test for every patient on that drug.

Match the test family to the specific question being asked, confirm the specimen is valid for that method, and treat the result as one data point that still needs clinical context.

A cartridge P2Y12 request arrives on a short-filled citrate tube from a patient with a platelet count of 62 x10^9/L. Put the correct workup sequence in order.

  1. 1. Match the test family to the actual clinical question

    An ADP-pathway question calls for an ADP-agonist method, LTA or a P2Y12-specific cartridge, not arachidonic acid, PT, aPTT, or platelet count.

  2. 2. Interpret the eventual result with full context

    Report the result against its own method-specific interval and note that it reflects the ADP/P2Y12 signal only, not adherence, absorption, drug interaction, or procedural bleeding risk.

  3. 3. Recognize the specimen as invalid

    Short fill changes the citrate-to-blood ratio, and the concurrent low platelet count can independently blunt an aggregation signal.

  4. 4. Apply the local specimen-rejection and recollection policy

    Follow the laboratory's defined criteria for tube fill and cell-count restrictions rather than releasing a result from a compromised draw.
