Why the routine panel cannot answer this, and what can

Platelet count describes platelet number, not drug-induced platelet signaling or aggregation response. PT and aPTT are plasma clotting assays that assess coagulation factor pathways and do not directly measure COX-1, P2Y12, or GP IIb/IIIa platelet effects. A normal platelet count with a normal PT/aPTT is compatible with almost any state of antiplatelet drug effect; it neither confirms nor rules out a pharmacodynamic response.

For antiplatelet questions, LTA pairs each declared agonist with a pathway-specific response and reports percent maximal aggregation: arachidonic acid interrogates COX-1/thromboxane signaling, while ADP interrogates P2Y12-mediated reactivity. Interpretation remains collection-, preparation-, agonist-, concentration-, and timing-dependent. Whole-blood impedance aggregometry measures a different signal entirely, an impedance change as platelets aggregate on wire electrodes in anticoagulated whole blood, and it retains the effects of platelet count and other whole-blood constituents on the result. Its output does not share LTA's intervals or cutoffs.

A cartridge P2Y12 assay, such as the FDA-cleared VerifyNow P2Y12 system, uses citrated whole blood and reports P2Y12 reaction units (PRU) rather than percent aggregation. PRU is not interchangeable with any LTA percent-aggregation interval. Any cartridge assay must be described by its exact current instructions for use (IFU): the specific instrument, cartridge, software version, accepted specimen, and reported unit, because these details govern handling, timing, reportable range, and flags.

A thromboxane B2 result, whether serum or a urinary metabolite, is a biochemical marker of thromboxane generation. Arachidonic-acid-induced LTA is a functional aggregation response. They measure different things and do not answer the identical question, even though both relate to the COX-1/thromboxane pathway. ADP-stimulated testing, whether by LTA or a P2Y12-specific cartridge, is the test family most directly aligned with residual ADP-mediated reactivity after a P2Y12 inhibitor; arachidonic-acid testing is the family aligned with COX-1/thromboxane pathway status after aspirin.

A laboratory reporting any of these methods must verify performance specifications for an unmodified FDA-cleared system, or establish them for a modified or non-FDA-cleared system, and maintain quality-control procedures that monitor the complete analytical process. No transferable numeric interval, PRU threshold, or LTA cutoff applies across laboratories; each laboratory establishes or verifies its own method- and agonist-specific interval for its intended population.

Choose the method by the pathway the question is actually about, and treat percent aggregation, impedance units, PRU, and thromboxane concentration as four different measurements that are not interchangeable.
