Read the field before naming the pattern

Assess red-cell size and central pallor in the well-spread monolayer. Thick areas distort apparent size, staining, and pallor, while the thin feathered field can exaggerate central pallor. A small lymphocyte nucleus is a practical reference for the usual mature red-cell diameter of about 7 to 8 µm: on the same field and magnification, compare its diameter directly against neighboring red cells rather than relying on memory or a separate reference image.

A microcyte is smaller than a normal red cell and a macrocyte is larger. A macro-ovalocyte is a large oval red cell and should be described separately from a round macrocyte. Anisocytosis is increased variation in size, not a synonym for microcytosis, macrocytosis, or poikilocytosis. Name a specific abnormality when it is identified.

MCV is an average cell-volume index. RDW represents variation in red-cell volume and supports, but does not replace, the smear assessment. Small and large populations can coexist while the MCV remains in range. Use the monolayer and a reference cell to decide whether the shift is uniform or variable.

A repeatable monolayer assessment

  1. Select a representative field

    Use the well-spread monolayer; do not judge pallor at the feathered edge.

  2. Set a size reference

    Compare cells with nearby normal cells and a small lymphocyte nucleus when available.

  3. Name observed patterns

    Describe size, pallor, variation, and distinct populations using approved local terminology.

  4. Correlate with CBC evidence

    Compare the film with MCV, MCHC, RDW, reticulocytes, flags, and histogram shape.
