Choose the reportable disposition

Use the guided-case evidence to make four linked decisions. First, classify the initial impedance count as reliable or unreliable. Next, identify the smear finding that changes the action. Then select the result, if any, that the laboratory can release. Finally, choose the documentation and communication action that respects local procedure.

Do not calculate a citrate multiplier from the numbers in this case. The alternate anticoagulant is intentionally unnamed because the tube, method, release format, and correction rules are laboratory-specific. The valid teaching action is to use the clump-free 178 × 10^9/L result only under the validated alternate-tube procedure. A manual estimate or a fluorescence channel would also require an approved method and a specimen that is analytically suitable.

An urgent communication threshold is a local policy decision, but reliability comes first. The unresolved 34 × 10^9/L number is not a verified critical platelet result. It must not simply disappear: when an unresolved result could represent severe thrombocytopenia, follow the local urgent-unresolved pathway for qualified review, prompt recollection or other validated resolution, and any required notification that clearly states its unverified status. Document the observed EDTA-associated clumping, the actions taken, and the method used for any authorized final result. Release the verified method result with the approved comment, not the most alarming preliminary number.
