Timing, calculation, and the variables that change the answer
Time and temperature matter because not every inhibitor acts the same way. A lupus anticoagulant typically interferes immediately, so LA mixing is read without incubation, within 30 minutes of preparation. Factor VIII (FVIII) inhibitors seen in acquired hemophilia A can instead be time- and temperature-dependent: the immediate mix may look corrected while a mix incubated for 2 hours at 37 degrees Celsius becomes prolonged as the inhibitor has time to act. Immediate correction alone does not exclude acquired hemophilia A, and a mixing study by itself cannot establish or exclude that diagnosis; incubated PT mixing is not performed, because the pattern the incubation is designed to reveal is an aPTT-reagent, FVIII-inhibitor phenomenon, not a PT one.
Several calculation approaches turn a mixed clotting time into an interpretation: visual comparison against the reference interval, percent correction, and the Rosner index (also called the index of circulating anticoagulant), calculated as 100 times (mixed time minus NPP time) divided by patient time. A multicenter field study found no single calculation method consistently separated deficiency from inhibitor across reagents and testing sites; percent correction performed well for PT, while aPTT interpretation needed method-specific confirmation and still varied by reagent and site. Because of that variability, a universal correction cutoff is not supported for any of these formulas; each laboratory validates its own decision limit and indeterminate zone against its own reagent, analyzer, and population, and states that limit as a local policy card, not a textbook number.
Specimen and drug review has to happen before any of this interpretation is trusted. Heparin and DOAC exposure can prolong clot-based assays and produce a false inhibitor-like pattern, so anticoagulant history has to be reconciled with the result before a noncorrecting mix is called an inhibitor. Hemolysis, icterus, and lipemia can also affect coagulation results in a way that depends on the detection method and reagent, with aPTT particularly susceptible to interference from these specimen conditions. If anticoagulant information is missing or interference is suspected, specific DOAC or low molecular weight heparin assays can help confirm or exclude that interference before the mixing pattern is interpreted.
Even a corrected mix is not the end of the story: a weak LA can be diluted enough by the 1:1 NPP addition that it appears to correct, so a corrected mix does not fully exclude LA, particularly when the clinical picture still suggests it. This is why mixing is a screening step inside a larger workup, not a standalone diagnosis, and why every laboratory's procedure defines what happens with an indeterminate or borderline result rather than leaving that judgment to the bench that day.
Read the immediate and, when indicated, the incubated result together, apply only the locally validated calculation and cutoff, and treat every noncorrecting pattern as inconclusive for a specific cause until drug interference is addressed.
