What this case supports, and what it does not
This case supports a documented conclusion: the emergency-department specimen set traveled 42 minutes unseparated at 23-25 C with no light cover, which is a plausible and guideline-consistent explanation for a low glucose and elevated lactate and ammonia, and a real light-protection excursion for bilirubin even though that result stayed inside its reference interval. Containment and identity were both intact.
This case does not support a quantified correction to any of the four results. No local analyzer, reagent, software version, or instruction-for-use was declared for this scenario, and stability numbers do not transfer across method or matrix without that documentation. The 5% to 7% hourly glucose figure is a guideline estimate under stated conditions, not a correction factor, and the same caution applies to every other number used in this case.
Method and local policy govern the exact answer in a real laboratory. Your acceptable time-to-separation window, your cold-chain band, your pneumatic-tube validation, and your exception-authorization list are all local-policy fields that no general guide can supply. When a claim here and your local procedure disagree, your local procedure governs.
The stability claim source hierarchy used here runs from the manufacturer's instructions for the exact assay, to your laboratory's own validation, to authoritative guidance such as CLSI PRE04 or CAP All Common Checklist requirements, when neither of the first two is available. CAP requires the laboratory to define and follow its own rejection and special-handling criteria for specimens beyond stability time limits; it does not publish a stability number itself. Guidance and accreditation standards support a controlled process; neither replaces the manufacturer's instructions or your documented validation.
Document what the transport record shows, name what it does and does not prove for each analyte, and let the assay-specific local procedure, not a general guideline number, decide the disposition.
