The courier is late and the tubes look fine
An adult emergency-department specimen set is collected at 09:00 for plasma glucose, whole-blood lactate, plasma ammonia, and total bilirubin. The courier reaches the laboratory at 09:42. The transport log shows 23 C to 25 C the whole way, no ice, and no light cover. At receipt the primary tubes are closed, the secondary bag is intact, and the patient identifiers on the tubes match the requisition. No hemolysis flag is reported. Nothing about the outside of this shipment looks wrong.
That is the trap in this scenario. Intact closures and matching identifiers tell you the specimen kept its physical integrity and its identity. They tell you nothing about what happened inside the tube while it sat unseparated at room temperature for 42 minutes. A correctly labeled, unbroken tube can still carry a biased result if the cells inside kept working during transport.
A receiving technologist checks three things at the bench: whether the specimen is biologically stable for the requested test, whether the container and its contents are physically intact, and whether the identity on the tube is secure. A specimen can pass two of the three and still be wrong for one test on the requisition.
By the end you will be able to read a transport log, predict which analytes on this requisition are vulnerable to a 42-minute unseparated ambient delay, and choose the bench action that a documented exception process actually supports, rather than the action that feels fastest.
