What this event does and does not tell you

PT-3 warrants a documented investigation. Determine the event status from the current provider report and its applicable minimum score; do not classify the event from one unacceptable material alone.

Review the unacceptable material, investigate the routine pathway, and use the documented patient-result review and corrective-action process when the evidence requires it.

One unacceptable PT result earns review and an investigation, not dismissal and not automatic unsatisfactory status.
