Basic mental model

A routine Romanowsky film can suggest inclusions, but a targeted stain demonstrates selected material. Compare number, shape, border, distribution, neighboring cells, routine appearance, confirmatory method, and diagnostic limits directly; one feature alone is insufficient.

Howell-Jolly bodies are usually solitary, round, sharply bordered DNA remnants on Wright-Giemsa. Pappenheimer granules are small irregular peripheral clusters that require Perls confirmation for iron. Basophilic stippling is diffuse fine or coarse blue granularity. Cabot rings are thin thread-like loops or figure-eight structures. Hb C-like crystals are dense rectangular or bar-shaped intracellular structures requiring hemoglobin characterization, not morphology alone.

A supravital stain is applied in vitro to living cells and directly shows Heinz bodies as one or more round blue-green inclusions. Wright-Giemsa does not show Heinz bodies directly; bite cells are indirect evidence after splenic removal of Heinz-containing membrane. Bite or blister cells plus oxidative context prompts supravital consideration. Angular clustered granules prompt Perls iron confirmation.

Inclusion evidence and stain-validity workflow

  1. Exclude overlay or artifact

    Refocus and compare repeated fields; route precipitate, overlay, or uncertain cell relationship to repeat preparation.

  2. Support intracellular material

    Require material within an intact RBC with reproducible morphology before a provisional inclusion label.

  3. Route parasite-like material

    Prompt qualified malaria review for suspicious parasite-like material; do not identify parasites by color alone.

  4. Confirm or hold

    Use Perls or supravital stain only with valid controls; failed controls invalidate the stain until correction, repeat, or escalation.
