Blast-like morphology with anemia and thrombocytopenia is concerning even without leukocytosis, and Auer rods support myeloid differentiation when present. An abnormal promyelocyte-rich presentation with dense granules, folded or bilobed nuclei, and multiple Auer rods raises urgent APL concern because it can require rapid PML::RARA confirmation and coagulation correlation. These are recognition and disposition findings, not an independently assigned final subtype.

Suspected APL coagulopathy and tumor-lysis assessment are related urgent contexts but separate processes: coagulation correlation plus rapid PML::RARA testing addresses the APL concern, whereas serial metabolic chemistry supports tumor-lysis assessment. Method and local policy matter. Analyzer model, software, flag rules, smear-review criteria, second-review triggers, specimen stability, allocation order, flow panels, reference intervals, component release or hold rules, and urgent communication rules must come from the validated local procedure. The values here are teaching data and are not universal thresholds for critical communication, tumor lysis, or treatment.

WHO fifth edition and ICC terminology should be named when classification conventions matter. Integrated interpretation combines morphology, flow cytometry, cytogenetics, FISH, molecular results, and clinical context. Release or hold only as the validated procedure directs, communicate the observed urgent concern, and let the integrated diagnostic process establish classification.
