The next laboratory action for this case is to select the local BCR::ABL1 diagnostic pathway, not to report a final disease label from morphology. The later e14a2 result establishes the transcript identity needed for standard monitoring. A low or negative assay result that covers only common transcripts cannot exclude an atypical BCR::ABL1 fusion. The report or referral pathway must make that limitation visible.

At month 12, an external laboratory reports 0.03% without documented IS calibration or conversion comparability to the local method. Preserve and release that valid external result with its laboratory and method context, document that it cannot be linked to the local trend, and route the report through the defined molecular-review recipient. Cross-laboratory trends require documented comparable transcript coverage, IS calibration or conversion, method performance, and sensitivity; do not create a false improvement by connecting noncomparable results.

For atypical transcripts, monitoring is personalized against that patient baseline rather than the standard International Scale. The treating team interprets response milestones and therapy consequences using current clinical guidance. When a platform, laboratory, conversion factor, transcript coverage, or software changes, retain the valid result, document the comparability limit, and follow the process for comparison, consultation, or unresolved-trend suppression.
