One specimen, six checkpoints
Every urine specimen moves through the same checkpoints regardless of why it was collected: positive identification and labeling at the moment of collection, transport to the laboratory, receipt with a documented date and time, storage or refrigeration while it waits, mixing immediately before an aliquot is taken, and finally analysis. A problem introduced at any checkpoint travels forward with the specimen; it is not corrected by careful work at a later step.
CAP requires the person collecting the specimen to positively identify the patient with at least two patient-specific identifiers before collecting the specimen; identification confirms who the patient is, and it happens first. CAP separately requires every primary specimen container, including a urine cup, to carry those same identifiers, and requires the collector to label the container in the patient's presence at the time of collection, before it leaves that presence, not afterward at a workstation; labeling is what ties the confirmed identity to this specific container. 42 CFR 493.1242 requires the laboratory to maintain written procedures, when applicable, for urine collection, labeling, storage and preservation, transport, processing, and specimen acceptability or rejection, and to document the date and time a specimen is received.
A specimen only carries evidential weight if two separate chains hold together: the chain that ties this container to this patient, and the chain that ties this container to a collection technique appropriate for the ordered test. CLSI PRE05 governs the second chain, the technical collection process for clean-catch, catheter, timed, and other urine collection methods; it assumes the first chain, patient identification, was already secured through the laboratory's identification procedure under CLSI PRE01. A break in either chain is not something the other can repair: a perfectly performed clean-catch collection from a misidentified patient is still the wrong patient's data, and a positively identified patient's specimen collected by a technique that does not match the order, a random void submitted for a timed collection, for example, still cannot answer the question that was asked.
Treat labeling, transport, receipt, storage, mixing, and analysis as one connected chain, because a break anywhere in it can change what the result means.

Positive identification and labeling
The collector positively identifies the patient with at least two identifiers before collecting the specimen, then labels the primary container with those same identifiers in the patient's presence at the time of collection, before it leaves their hands.
Collection by the correct technique
The collection method matches the ordered test: clean-catch midstream, catheter port, timed interval, or another PRE05-covered technique, following the current instruction sheet.
Transport
The specimen travels to the laboratory within the interval and temperature the method and local policy require, protected from light if the requested analytes are light-sensitive.
Receipt
The laboratory documents the date and time of receipt, matching it against the reported collection time to calculate elapsed time.
Storage or preservation
A specimen that cannot be analyzed immediately is refrigerated, preserved, or returned to room temperature under a written procedure, since preservative performance is analyte- and method-specific.
Mixing and analysis
The specimen is mixed by the laboratory's validated technique immediately before an aliquot or sediment preparation is made, so the portion tested represents the whole specimen.
