A number with no red flag
A basic metabolic panel comes back on a routine morning run. Sodium, chloride, CO2, and glucose all sit inside their reference intervals. Potassium reports at 6.8 mmol/L, a value high enough to be an immediately life-threatening finding if it reflects the patient's actual blood chemistry. Nothing else on the panel looks unusual.
You pull the run record before you do anything else. Both levels of quality control passed for this chemistry panel. The potassium electrode's calibration is current. The instrument raised no analytic error flag on this specimen: no clot detected, no insufficient sample, no absorbance or potential outside its expected operating window. The delta check then held the result for manual review rather than autoverifying it for release.
A chemistry analyzer does not print a number out of nowhere; it moves a specimen through a fixed sequence of physical steps, and each step can go right or wrong independently of whether quality control passed for the run as a whole. A passing QC result tells you the system is working in general. It does not tell you this one specimen was collected, transported, and measured without incident.
