What the pattern supports and cannot establish

The case supports G3a with A2 albuminuria, PCR 210 mg/g interpreted against the laboratory's local PCR interval, microscopic RBC corroboration under the illustrative local microscopy rule, and granular-cast renal context. It does not establish cause, disease diagnosis, or chronicity. Correlate the pattern with the ordering clinician.

Dipstick cutoffs are manufacturer- and lot-specific, and the blood pad cannot distinguish hemoglobin from myoglobin. Cast recovery depends on preparation and urine conditions. Exact acceptance, repeat, reflex, and escalation criteria are local policy decisions under the CLSI PRE05 framework.

Release the pattern, limitations, and policy-based action together. The numeric microscopy boundary is illustrative and local; strip grade, PCR interval, confirmation, and escalation decisions depend on the laboratory's validated methods and policy.
