Lyme serology runs in two tiers
Borrelia burgdorferi is hard to find in blood, so Lyme testing reads the antibody response. Serology can stay negative for 4 to 6 weeks after infection.1
Standard two-tier testing starts with a sensitive EIA. A negative first tier ends the test. A positive or equivocal first tier reflexes to immunoblots, read by how long the patient has been ill.2

Modified two-tier testing replaces the blots with a second EIA that uses different antigens, in pairs cleared together by FDA. Positive or equivocal results on both tiers read positive.2 A positive result shows antibodies from recent or past infection. IgG lasts months to years after cure, so serology cannot follow treatment.2
An IGRA reads T-cell memory
An interferon-gamma release assay (IGRA) measures the interferon-gamma that T cells release when blood meets Mycobacterium tuberculosis antigens. Bacille Calmette-Guérin (BCG) vaccination does not affect it. It shows infection and cannot separate latent infection from active disease.3
The whole-blood ELISA format runs a Nil tube, two TB antigen tubes and a Mitogen tube. Nil is subtracted from each of the others.4
- Nil must be 8.0 IU/mL or less. Above that, the result is indeterminate.
- Positive when either TB tube minus Nil is at least 0.35 IU/mL and at least 25% of the Nil value.
- Negative when both TB responses fall short and Mitogen minus Nil is at least 0.50 IU/mL.
- Indeterminate when Mitogen minus Nil is below 0.50 IU/mL and neither TB response is positive.4
An indeterminate result means the test did not work, often from a failed mitogen response or a high Nil. It says nothing about infection. A repeat IGRA or a tuberculin skin test can resolve it. The laboratory reports the qualitative result together with the numbers.3

- Nil is 0.20 IU/mL, below the 8.0 IU/mL limit.
A high Nil makes every other tube unreadable, so Nil is checked first.
- TB1 − Nil = 0.28 − 0.20 = 0.08 IU/mL. TB2 − Nil = 0.31 − 0.20 = 0.11 IU/mL. Neither reaches 0.35 IU/mL.
A positive antigen response settles the result whatever the mitogen shows.
- Mitogen − Nil = 0.40 − 0.20 = 0.20 IU/mL, below 0.50 IU/mL.
With no positive antigen response, the mitogen shows whether his T cells could respond at all.
- No antigen response is positive and the mitogen failed, so the result is indeterminate.
A failed positive control means the negative antigen tubes prove nothing.
- Report indeterminate with the four values. A repeat IGRA or a skin test can resolve it.
The reader needs to know the test gave no answer.

The clue that settled the Lyme result is 10 weeks of illness, which leaves the IgG blot as the only one read. For the IGRA, a single TB tube meeting both limits made the result positive.
