Debrief: what this method does and does not tell you

A repeatable search method: mix and prepare the specimen so the deposit represents it, scan systematically at low power before any high-power stop, identify structures from their deciding visual features, use bright-field, phase contrast, or polarization only to reveal an ambiguity, and quantify in the laboratory's own reportable terms. Manual urine microscopy has no single reference method; detailed preparation and scanning steps vary by validated local procedure, and that variability is expected, not a flaw to correct with a universal recipe.

What is not supported here is any specific centrifugation condition, resuspension volume, report unit, reference interval, or automated-analyzer threshold. Those depend on the laboratory's instrument, reagent, software version, and its own verified or established performance specifications, which CLIA requires before patient results are reported on a nonwaived test system. An analyzer Review flag means an operator looks at the result; whether that leads to manual microscopy is a local reflex decision.

Consistency among the people performing the exam matters as much as the procedure itself. Laboratories accredited by the College of American Pathologists are required to evaluate the consistency of morphologic observation among personnel performing urine sediment microscopy at least annually, with the laboratory director or designee setting the acceptability criteria and documenting corrective action for anyone who drifts from group consensus.

A defensible sediment result is a representative preparation, a systematic scan, a feature-based identification, a count in the laboratory's own terms, and a documented decision about whether it needed another look.
