One field is not the specimen

A bench technologist scans a urine sediment slide, sees three squamous epithelial cells in the first high-power field, and reports the sediment as unremarkable. Two fields over, a cluster of erythrocytes and a hyaline cast sit just outside where the first glance landed. The slide did not change. The scan did.

Urine sediment microscopy has no single reference method. Different laboratories validate different centrifugation conditions, loaded volumes, and scan patterns, and CLIA requires each laboratory to verify or establish the performance of its own procedure before reporting patient results. What stays constant across every valid method is the discipline of the search: a representative preparation, a fixed low-power scan of the whole area before any high-power stop, and a decision rule for when a result needs another set of eyes.

The discipline is: prepare it so the deposit represents the specimen, scan it so distribution and rare elements are not missed, quantify it in the laboratory's own terms, and know when to stop and ask for review. It does not hand you a centrifugation recipe, a report unit, or a reference range, because those are validated local method values that vary by instrument, reagent, and population.

An isolated impressive field and a representative scan are not the same finding, and only the second one belongs in a report.
