What the sediment supports, and what it does not

The guided case supports a bounded set of observations: RBC and WBC counts modestly above the local reference interval, many squamous cells raising a collection-quality question, and small refractile structures with repeatable walls and budding classified as probable yeast. None of this supports a diagnosis of urinary tract infection, candiduria, or any other clinical condition, because manual urine microscopy is a morphological and semiquantitative examination and does not by itself establish organism identity, viability, or antimicrobial susceptibility.

What depends on method and local policy is substantial. The exact preparation, reporting scale, and review process for a laboratory's sediment examination come from that laboratory's method verification and manufacturer instructions, not from a textbook. The laboratory's written procedure is the controlling document, and it must state collection and acceptability criteria, the microscopic examination itself, result interpretation, limitations, controls, reference intervals, and the result-release process. Reportable units, reference intervals, and any critical value are locally established or verified for the exact method, population, and unit in use; no universal sediment reference interval is declared here.

Unexpected organisms, uncertain renal tubular cells, suspected parasites, and cells raising concern for malignancy sit outside routine image-atlas identification. Those findings go to the laboratory's validated escalation, consultation, or referral pathway rather than being resolved by comparing a picture to a look-alike pair. Name the two features that drove the call, keep the claim bounded to what morphology and correlated chemistry actually show, and route anything outside that bound to the people and processes the local procedure names for it.
