Sort the field and defend the call

Working image-atlas practice presents a set of authentic, rights-cleared images one at a time and asks the learner to sort each into one of three bins: confident identification, probable category, or requires review, then name two visible features that justify the sort. A confident identification means the structure shows unambiguous, textbook features with no competing look-alike explanation in view. A probable category means the leading features point one direction, as with the guided case's refractile structures and their budding, but a second confirmatory feature is thin or the field is imperfect. Requires review covers uncertain renal tubular cells, suspected parasites, cells raising concern for malignancy, and any other structure outside routine image-atlas identification, all of which route to the local escalation pathway rather than a bench-level call.

The exercise below works the reasoning sequence for the guided case's ambiguous refractile structures step by step, in the order a bench scientist actually uses it: note the size relationship to nearby RBCs, refocus through multiple planes to examine the wall, look specifically for budding or pseudohyphae, correlate with the chemistry panel, and only then assign a classification category. Skipping ahead to a category before working the earlier steps is the most common source of an over-called or under-called sediment finding.

The three-bin sort and two-feature justification exist because a laboratory report communicates a bounded observation, not a certainty. Writing down which two features drove the call also creates a record a colleague or reviewer can check, which matters more for a probable or requires-review classification than for a confident one.

Ordering exercise

Put these steps in the order a bench scientist should use them to classify the guided case's small round refractile structures (8-12/HPF) as probable yeast, RBCs, or oil droplets.

  1. 1. Assign a classification category

    Classify the structures as confident, probable, or requires review and record the two features used to decide.

  2. 2. Look for budding or pseudohyphae

    Search specifically for a narrow-based bud or elongated pseudohyphae, the strongest morphologic evidence for yeast.

  3. 3. Note size relative to RBCs

    Compare the refractile structures to the intact RBCs already visible in the same field.

  4. 4. Correlate with the chemistry panel

    Read the trace leukocyte esterase and 2+ blood alongside the microscopic finding as supporting, not confirmatory, evidence.

  5. 5. Refocus through multiple planes

    Rack through focus to examine whether each structure has a defined wall rather than a flat, featureless outline.
